Transition Partnership Program



 
BRIDGING THE GAP FROM SCHOOL TO SUCCESS
A collaborative program between
Modoc County Office of Education
and
California Department of Rehabilitation

The goal of TPP is to prepare young adults for the world of work.

Program Services
  • Vocational Counseling
  • Career Planning
  • Job Development
  • Teach Job-Seeking Skills
  • Job Coaching
  • Help pay for college or technical school
  • Purchase necessary tools, equipment and supplies
  • Pay for necessary clothes for interview or job
  • Referral to other agencies offering support services
Eligibility - Persons qualifying for TPP must:
  • Be enrolled in a high school program in Modoc County
  • Be at least 16 years old and / or a junior in high school
  • Have a verifiable disability (physical, learning or emotional)
Support Services
  • Field trips to career training facilities such as Job Corps and colleges
  • Provide vocational assessments to match aptitude and interests with career options
  • Assist in locating training programs for developing vocational skills
  • Provide tutoring skills, as needed, to assist students in reaching their career goals
  • Assistance with college applications and financial aid forms

Questions & Answers about TPP

What does the program cost?
    Most services are not based on income and do not involve a fee.

What happens if the student moves outside Modoc County?
    Once a student is a client with the Department of Rehabilitation, the case may be transferred to other areas within the state.

Does a student have to be in Special Education to qualify?
    No - people with other types of disabilities can also qualify.

What are some examples of other qualifying disabilities?
    Chronic asthma, heart trouble, orthopedic problems, acute vision or hearing loss, and substance abuse

What if the student is already enrolled in college or technical school?
    The student may still be eligible for assistance with books, tuition, special tools or transportation.

How long can someone be enrolled in the program?
    There is no time limit.
